Etrigan's a demon, so I guess one could argue he either doesn't get the human problem with "lack of clothing" and "being in weird positions", or he doesn't care.
Probably the second one. He's from Hell. He enjoys our suffering!

And I'd say it's the last, he understands weird positions enough to french kiss his host.
